Earn Free Food -- Terms & Conditions

The terms and conditions are as follows:

• Applies to customer referrals given after May 13, 2009.
• The new referral program replaces prior referral offers.
• If two customers refer the same person, only the one from whose list the sales appointment was first made will receive the credit.
• All Five Star account payments must be current in order to qualify for the Free Food Order.
• After a customer refers 5 new customers and receives their Free FS-1 Food Order, they continue to earn referral credits as if they were starting the program originally. (ie. They receive $100 for referrals 6-9 and another Free Food Order for their 10th referral and so on)
• This promotion is separate and distinct from the Health Professionals Partnership Program. (HPPP) HPPP partners are ineligible for this promotion.
• $100 Referral Credit Vouchers cannot be used on half orders. Customers must place a minimum FS-1 Menu to redeem their $100 credit voucher(s).
• Free FS-1 Food Menus will be planned with customer by phone.
• $100 Referral vouchers are not redeemable for cash and cannot be credited to current accounts.
